Email Template The template used for email layout.  
Redirect Route The url URL the user will be redirected to on a successful form submission.  
Button Label  The text on the submit button.Default is: Send 
Valid Upload Types Comma A comma-separated list of files extensions you would like to accept file extensions acceptable for upload on this questionnaire, for . For example: .jpg,.png,.pdf,.doc  
Allow Multiple Upload Files Turn this off if you want to only accept one file uploadDetermines whether multiple files can be uploaded to one form. If disabled, only one file upload will be accepted.
